In New Jersey and nearby areas:

Many of the ferries are not car ferries. The Staten Island ferry, while equipped for vehicles, stopped carrying them a while ago, and fast ferries such as Seastreak...

By "proper engineering" I meant working out good bus and perhaps even trolley routes to and from ferries and planes, on the islands themselves, and better management and construction of the ferries...

Well, I haven't come across any, and my opinion that "it kills whales" stemmed from this video about "similar" ferries killing whales in the Canary Islands...
